This form is for repeat orders of lenses you have ordered already through Eyes on Buderim.  If you are wanting to change to a new contact lens or you are a new patient you will need to contact us.

Note 1.  Having your lenses sent to you.

CooperVision lenses are sent directly from CooperVision to your address at no extra charge.  They will send to a street address but not a PO Box.  Other brands such as Johnson & Johnson, Alcon and Bausch and Lomb can still be sent but there is a $10 charge for the post bag.

If you wish us to charge your health fund directly, you will need to pick up your lenses in person from Eyes on Buderim and bring your health fund card.
